     5.  This Certificate EXPIRES on December 31, 2009.  Please redeem before this date.

      6.  This Certificate is VOID if the Red Serial Number on the top right corner of the application has been removed or altered in any way.

You will receive your FREE $300 Gas Cards Vouchers and your $1,000 Fast Food Restaurant coupon book by U.S. Mail within 3-4 weeks of mailing the completed Reward Certificate.
